📌 Definition
• Parainfluenza viruses (HPIV 1-4) are RNA viruses in the paramyxoviridae family, causing respiratory tract infections (notably croup, bronchiolitis, and pneumonia), especially in infants and young children.
📌 Causes & Transmission
• Spread via respiratory droplets (coughs/sneezes) and direct contact
• Four main types (HPIV-1 to HPIV-4) — each linked to different clinical patterns
• Outbreaks common in autumn and winter, especially in crowded settings (e.g. nurseries)
📌 Risk Factors
• Highest risk: infants, young children, immunocompromised, elderly
• Close contact environments (daycare, hospitals)
📌 Pathophysiology
• Virus infects epithelial cells in the upper airway → inflammation and mucus → airway narrowing, classic symptoms
• Memory hook: Barking cough = airway swelling
📌 Differential Diagnosis
• Symptoms can mimic RSV, influenza, adenovirus, human metapneumovirus—lab testing often needed for confirmation

📌 Diagnosis
• Mainly clinical (esp. croup: barking cough, stridor)
• Confirmation: rapid antigen test, PCR, or viral culture on nasopharyngeal swab
• Chest x-ray if severe
📌 Management
• Supportive care is the mainstay: fluids, rest, paracetamol/ibuprofen for fever
• Severe cases: Hospitalisation, oxygen, nebulised adrenaline, steroids (for croup)
• Antibiotics not indicated unless secondary bacterial infection suspected
📌 Complications
• Severe croup, bronchiolitis, pneumonia
• Otitis media (middle ear infection)
• May exacerbate underlying lung disease
• Rarely: respiratory failure in high-risk groups
📌 Prognosis
• Usually excellent — most recover fully
• Watch for deterioration in infants, elderly, or immunocompromised
