Direct composites provide the dentist with the ability to provide excellent aesthetic results in a minimally invasive manner. In this webinar, Dr. Serhat Köken, specialist in aesthetic dentistry, demonstrates how to achieve excellent aesthetic standards when placing composites in anterior teeth, illustrated with clinical cases. The clinical guidelines for colour perception are really useful in daily practice. Morphology is the most essential aspect while doing anterior restorations and is visualised better with photography. A good camera with the correct lenses and illumination is necessary. A twin flash is easier to evaluate texture and line angles than a ring flash. A cross polarisation system is very useful in shade selection because the reflection on the surfaces is eliminated. It can be combined with the composite button try technique. Black and white images help in determining the value, while saturated image help choosing the right chroma. Photography can even visualise the opalescence and the more transparent incisal halo. Important points that define the morphology are the outline, long axis, location of contact points, the zenith points and line angles, the incisal embrasures, emergence profile and grooves and ridges. These features should always be kept in mind. It is explained how the hue, value and chroma are varying between the different shades. Chroma is very important in the dentin, while enamel will determine the value of the restoration. The mamelon design should be carefully studied while the incisal third is created, to mimick the natural teeth. It is shown how the tooth should be layered to mimick the dentin and enamel structure of the natural tooth. Finally, it is explained how the texture influences the colour perception and how texture and gloss should be created in the final steps of the restoration. Handling composites correctly can provide excellent aesthetic outcomes, which is illustrated with beautiful clinical examples in this lecture.
