A joint investigation by this programme and the Independent newspaper has found hundreds of claims for compensation in the last decade, after the death or injury of babies at maternity services run by Nottingham University Hospitals Trust. (Subscribe: https://bit.ly/C4_News_Subscribe)
It stands accused of unsafe care and neglect, and of failing adequately to investigate births where the baby or mother died or was harmed.
Accurate records were not kept and grieving families were left in the dark. One whistleblower has told us there's a culture of fear in one of the trust's maternity units.
