This was one of the most dramatic thoracic cases I’ve seen in a while. A dog under anesthesia with massive pleural effusion, widespread irregular pleural enhancement, lung collapse, lymphadenopathy, and a small pneumothorax.
There’s no obvious foreign body, but this looks like severe pyogranulomatous pleuritis, possibly from a migrating plant awn or other infectious/inflammatory cause.
In this video I break down:
What the pleural changes actually mean
Why the lungs look collapsed and triangular
How to think about infectious vs inflammatory vs foreign body causes
Why a median sternotomy may be the right next step
If you’re a veterinarian, tech, or just curious about veterinary medicine — this one’s wild.
I’m Dr. Peter Noël, board-certified veterinary radiologist.
