Scoliosis in Children: Symptoms and Treatment Options

When patients think of scoliosis, they often associate it with children because it is most commonly diagnosed in adolescence. The greatest population of patients with scoliosis is actually adults and related to age. Every decade of life, the instances of scoliosis increase in that patient population.

In the adolescent form, scoliosis can be multifactorial. Adolescent idiopathic scoliosis is when there is no single cause associated with the scoliosis. Even though scoliosis is diagnosed in the adolescent stage, we believe it occurs younger in life in the juvenile years. Something causes the curve to develop, and in the growth stage, it starts progressing and is diagnosed.

There are other types of scoliosis that affect children diagnosed as infantile scoliosis or juvenile scoliosis, such as neuromuscular scoliosis, congenital scoliosis, and idiopathic scoliosis. The biggest trigger for progression is growth. As curves become bigger, they are more likely to progress as patients grow.

There is no way to predict how much a curve will progress during adolescent growth spurts, but we do know this is where they progress the most. We believe this affects girls earlier than boys because they go through an earlier and faster growth spurt.

Scoliosis needs a customized treatment approach created based on the patient's age, condition type, severity, and where the curve is located, so as curves get bigger, the treatment options become more intense in terms of how we're dealing with it. Normally in conservative treatment approaches, we want to treat scoliosis cases in a much smaller size. Conservative treatment approaches are designed to stop curves from progressing to surgical standards.

However, traditional treatment approaches tend to have fewer treatment options for smaller curves and to wait for curves to become bigger, then try to treat them when severe. Modifying your treatment approach to meet your needs and the unique challenges of that person, be it an infant, juvenile, or adolescent, is also very important. As we know, the younger a patient has surgery, the more likely they are to need multiple surgeries over their lifetime. which can have different types of complications.

The first sign of scoliosis in children is asymmetry, unlevel shoulders, hips, waist, and rib rotation. When patients tend to go through growth spurts, the curves can progress very quickly, so do not take postural asymmetry lightly, as it is not normal in children.
