Описание: A premature baby left to die alone in a room intended for dirty bed pans, a mother dying of a 'catastrophic hemorrhage' after her deterioration was wrongly assumed to be a mental health issue. Just some of the failings revealed by a review of maternity care at the Royal Oldham and North Manchester hospitals. A damning report obtained by a Freedom of Information request - laid bare chronic staff shortages, bad medical decisions and poor staff attitude - all of which led to risk and harm to mothers and babies.
