This form is primarily used by non-institutional providers, such as therapists, physicians, midwives, dentists, and nurse practitioners, to bill Medicare for patient services.
While the Administrative Simplification Compliance Act generally requires Medicare claims to be submitted electronically, the CMS-1500 form is used in specific situations. These include cases where the HIPAA claim standard does not permit electronic submission, when disabilities among a provider’s staff prevent the use of electronic claims, or other rare and unanticipated circumstances.
In such instances, providers—such as physicians, suppliers, and other healthcare professionals—must rely on the CMS-1500 form, which includes detailed sections for patient and insured information, physician or supplier information, diagnosis codes, procedure codes, and charges.
