Mention *Accenture joining date* as the current employment start date.
Bank details are optional but *PAN number is mandatory*.
Indicate whether you had PF and EPS accounts in previous employment by checking your EPFO PF statement**.

If pension balance is zero, select No for EPS membership.
If a positive balance exists, select *Yes*.

PF Details Based on Previous Employer Type

*Un-exempted Companies (PF details available on EPFO portal):

Enter most recent company name, *UAN*, *PF account number*, joining and exit dates.
Leave scheme certificate fields blank.
Mention NCP days (gap between last working day and Accenture joining) or enter zero.

Exempted Trust Companies (e.g., TCS, Infosys, HCL, Wipro):

Provide company name, *UAN*, *EPS account number*, joining and exit dates.
EPS details can be found in salary slips or PF statements.
Leave scheme certificate fields blank.
Enter NCP days or zero if unknown.

International Worker section should be marked *No if holding an Indian passport.
The form must be *signed on Day 1 (Date of Joining).
