The unit was operating under significant pressure, with documented staff shortages, infrastructure concerns and rising neonatal deaths. As baby collapses increased, suspicion began to focus on one nurse. Yet earlier reviews, including those by the Royal College of Paediatrics and Child Health, did not find evidence of deliberate harm.
We break down how the police investigation developed, how expert medical evidence shaped the prosecution case, and how those conclusions were presented to a jury. We also explore why the Lucy Letby evidence has since been challenged, including questions around forensic proof, eyewitness testimony and the role of circumstantial medical interpretation in securing convictions.

Our guest, Dr Svilena Dimitrova, a consultant neonatologist and specialist in patient safety, now advising the defence in an application to the Criminal Cases Review Commission, argues that the Lucy Letby convictions may represent a miscarriage of justice. She examines the medical evidence case by case, including the insulin poisoning allegations and the death of Baby O, and explains why she believes system failure, clinical error and misinterpretation of data may offer alternative explanations.
