Certain applicants can obtain an EIN directly from the IRS via their online EIN Assistant website application system, or via a paper form submitted to the IRS

Attorney Burton also explains how THERE IS a filing fee and annual fee at the state level, payable to the state where you organized (formed) your LLC. The LLC filing fee and annual report fee vary from state to state, so it is important to check on this information with the office in your particular state where the LLC Articles of Organization are filed. In Wisconsin, LLCs are organized by filing Articles of Organization with the Wisconsin Department of Financial Institutions. Any fees discussed in the video were the current fee shown by the state in question on their website at the time this video was recorded, but the filing fees and annual report fees in each state may be changed by the state at anytime, therefore it is best to check on the current filing fee and annual report fee directly with the state office where you plan to form your LLC for the latest information.
