Arc flash incident video illustrates consequences of not recognizing an increase in work task Risk Level.
In 2010, the Panel Manufacturer personnel were carrying out the installation of a new energy meter and related wiring works in the incomer metering panel.
Due to inadvertent touching of a neutral wire with the incoming live terminals, flashover occurred causing first-degree burn injuries to one person in the face and left hand.
Root causes of the incident:
Risk Assessment for the project was not
conducted.
Uninsulated tools were used.
Overconfidence of the worker, as similar jobs were done
earlier in ten locations.
