Established in 1992, North Central Family Medicine is a Federally Qualified Health Center.
Our mission is to improve the health of the communities in the designated service area by providing quality comprehensive healthcare that is accessible and affordable through collaborative efforts. Our motto is “QUALITY AND AFFORDABLE HEALTHCARE.” Along with its many other primary health care services, North Central is very proud of its new Centering Pregnancy Program®, which allows pregnant women to participate in group sessions and share their experiences with each other.

Retail Pharmacy:

Our 2 retail pharmacies are open to the public!

Community Medicine Pharmacy is a price non-profit pharmacy established in 2007. We are a Federally Qualified Health Center Pharmacy. This means we can purchase medications at much lower prices and pass those savings to you.

Accepting tax-deductible charitable contributions

Community Medicine Foundation, offsite Patient Treatment Care Fund was created by our Board of Directors and established as a 501(c)(3) organization. The purpose of this fund is to assist uninsured North Central Family Medicine patients in obtaining medications for healthcare critical needs and to obtain access to medical specialty services (urologist, gastroenterologist, radiology, orthopedic, and ob/gyn services, just to name a few).

Your charitable contributions will go to the Community Medicine Foundation Fund, Offsite Patient Referral Care Fund.

Please write checks out to: Community Medicine Foundation, and in the memo line please specify: Offsite Patient Referral Care Fund.

While Community Medicine Foundation, dba North Central Family Medicine does provide primary general family practitioner services, our uninsured patients are often denied access to critically needed medical services.
