For the CELPIP task 1 writing, much like the IELTS General Task 1 letter, the best thing is you don't have to use any addresses, dates, or subject lines. Here's what you need!
Informal:
Dear Bob, (just the name of the friend)

Sarah (your first name only)
Formal:
Dear Sir/Madam,
OR
Dear Mr. Rockwell,
In CELPIP, you want to personalize greetings, hence, we use the above instead of TO WHOM IT MAY CONCERN. Also, please check the question. Sometimes, it mentions there what you exactly need to start with.
