香港有超過53萬名殘疾人士，當中超過七成為50歲或以上。研究顯示，他們比非殘疾人士更容易出現繼發性疾病、醫療併發症和身體機能早衰的問題。為支援患有末期或危及生命疾病的殘疾人士面對各種健康挑戰，由香港賽馬會慈善信託基金主導策劃和捐助的「賽馬會安寧頌：融和篇」將結合跨界別的團結力量，透過不同服務模式聯繫社區及醫療系統，以推動殘疾人士紓緩治療和臨終關懷服務在香港的持續發展。

[Jockey Club End-of-Life Community Care Project: Unison]

Hong Kong is home to over 530,000 persons with disabilities (PWDs), approximately 70% of whom are aged 50 or above. Studies show that PWDs are more susceptible to secondary conditions, medical complications and premature ageing. To support PWDs with terminal or life-threatening illnesses who are facing such challenges, The Hong Kong Jockey Club Charities Trust initiated and funded the “Jockey Club End-of-Life Community Care Project: Unison”, a collaborative effort across multiple disciplines aimed at enhancing the capacity of professionals delivering palliative and end-of-life care, as well as raising public awareness through various training sessions, workshops and community activities.
