Best Tips & Tricks for surviving in Hardcore Diablo 3 (Also if you're struggling on Softcore)

General:
ALWAYS use a cheat death passive (+ potentially follower)
ALWAYS use the "freeze fix" to prevent game crashes as much as possible (see here:    • Diablo keeps crashing/freezing for you? He...  )
NEVER use your best items for farming, instead farm with your second best set of equipment unless these are items that are imperfect / easily replaceable
Don't augment anything before you are ready to push unless
a) you often play DPS in groups
b) you have way too many gems lying around
c) you get items you won't replace easily/ever
d) you know you will only play one or a few builds the entire season
BEWARE of Dune Dervishes ("spinners", often spawned as minions by Nemesis Bracers) & Sand Dwellers: they can reflect your projectiles and you can oneshot yourself or your teammates. There is NO WAY to survive any of your own projectiles from these guys
ALWAYS pay attention to which monster types are present in your map. Beware of ground effects (e.g. Accursed) or explosions (e.g. Grotesques, Fallen Lunatics) or other dangerous monsters (Noxious Guardians) that can kill you even when you defeat them. Also pay extra attention to elite affixes like Waller (don't get stuck in a corner), Molten (don't run into explosions) and Knockback/Vortex (when you are not immune to crowd control)
ALWAYS use a mobility/immunity skill in your build
Build your character more tanky than usual to avoid proccing / dying, e.g. add more 200-400 more vitality in paragons or replace very greedy offensive choices with defensive ones
If you build is using Stone Gauntlets, beware of its negative effects and make sure to NEVER attack when you have 5 stacks and your crowd-control immunity is missing or else you will be stuck in the animation for multiple seconds

Activity-specific tips:
For "easy mode" activities like T16 / bounties, make your character extra tanky and take things like Molten Wildebeest if it means no noticeable loss of farming efficiency to increase your chances of surviving a disconnect
When in groups: best case go on voice comm to call out lags / disconnects to your team so they can protect you
For general GR and EXP farming, choose a difficulty that allows you to effortlessly destroy anything. Dead enemies are no threat to you and if you can oneshot everything in sight after you had a cheat death proc, you can stay safe until it's back up
When you proc your cheat death, there is often no need to risk anything and you can return to town to play it safe. Only press forwards if you are very confident in your own ability to clear the run easily and can sharpen your senses to the main sources of danger for the duration
When pushing: rather put in a few more keys to restart a run when it goes downhill and play it safe instead of trying to fight against terribly dangerous monster types and elites

Class-specific tips (had to cut it a bit to fit the YT description maximum, see in the pinned comment instead):

Barb:
Keep up Band of Might, always!

Crus:
Play around Akarat's Champion - Prophet and always have Steed Charge or Iron Skin - Flash.

Monk:
Always have either Dashing Strike or Serenity (or both).

DH:
Take Elusive Ring until you feel comfortable with Convention of Elements.

Necro:
Blood Rush costs life, be aware! Also use Leech curse to heal up when in danger before moving.

WD:
Be aware of crowd control effects and always save Spirit Walk's cooldown when you can.

Wiz:
ALWAYS look at Halo of Karini buff on your bar and make sure you stay away from enemies enough to activate it.
