Hannah Capon gets together with Dr Tom Harcourt-Brown to chat about misinterpretations of common clinical signs of arthritis. Using a fictitious case study, they describe a dog that appears to have severe arthritis-related disabilities. However, as they look closer at some telltale signs, it turns out he actually has a condition that is affecting the function of his nervous system and is in fact not in pain. Tom explains how he would distinguish between neurological deficits and mechanical/orthopaedic problems and how management and treatment will differ depending on the presence or absence of pain.

Tom Harcourt-Brown graduated from the University of Cambridge in 2002 and worked in a general small animal practice on the outskirts of London before returning to his home town of Harrogate in Yorkshire to work in his parents' practice between 2003 and 2007. During this time he studied for his RCVS certificate in diagnostic imaging, which he was awarded in 2007. Following his employment in Harrogate, he took up a post as a senior clinical training scholar in neurology and neurosurgery at the Queens Veterinary School Hospital in the University of Cambridge. He passed the examination for the European College of Veterinary Neurologists in September 2010. Tom has been at Langford Vets since 2010, initially running the neurology referral department on his own but now part of a growing team of senior neurologists, residents in training and one full time neurology nurse catering to one of the busiest referral departments in the UK. He is primarily responsible for seeing and supervising cases referred to the hospital but also spends time teaching the residents and final year students on rotation, as well as being one of the examiners for the European Diploma in Veterinary Neurology. In 2020 Tom was awarded a fellowship of the Royal College of Veterinary Surgeons for meritorious contributions to clinical practice.

Canine Arthritis Management (CAM) is a veterinary driven initiative, set up by vet Hannah Capon who was becoming overwhelmed with the number of dogs she was having to put to sleep having "gone off their legs". This was often seen by the owners as a sudden incident, when in truth that dog had probably been suffering in silence for a long time prior to that day. In fact, we believe that as many as 1 in 5 dogs in the UK, and 80% of dogs over the age of 8, will have some degree of osteoarthritis. Unlike humans, dogs are unable to express their pain in words. Chronic pain is no doubt something most people will understand and empathise with, but it can be difficult to tell when a dog is suffering in the same way. We want to challenge the preconceived notion that "just getting old" or "slowing down" should be accepted in our four-footed companions. Ageing in itself is not a disease! Here at CAM we think that by changing owner, vet and public perception of arthritis, we can improve and extend the lives of dogs.
