HAZOP and LOPA make a fundamental assumption that the plant is "maintained" as per the manufacturer's recommendation which is often not a reality. This new approach accounts for interactions between equipment and/or human interfaces with effects on process safety, to predict the likelihood of a major accident. This methodology changed the perceptions of challenging adequacy of controls and paved innovative ways of presenting business case for managing risk to ALARP levels. In an Australian LNG facility, it had significant impact on safety features design, and has established way forward for changing safety controls in fourteen similar plants across the world.
