Old Tandy 1000 RSX (386sx25) running DeskMate and the Music program that allows you to use 3 instruments to write sheet music with. These are the samples that came with this version of DeskMate (3.0x). Yes, the date displayed is correct. I used a camera to take screen shots and recorded the output with my main computer a while back. Enjoy the nostalgia.
