Prostate Cancer MRI – PI-RADS 5
S9 Teleradiology | Educational Post
🧠 Key MRI Findings
• T2W: Focal hypointense lesion in the peripheral zone with loss of normal glandular architecture.
• DWI/ADC: Marked diffusion restriction (very high signal on DWI, very low on ADC). ( b value 1500)
• DCE: Early focal asymmetric enhancement.
• Margins: Irregular with possible capsular involvement, increasing suspicion of extraprostatic extension.
These features together make this a PI-RADS 5 lesion — the highest category of suspicion.

2️⃣ PI-RADS 2: Low likelihood
3️⃣ PI-RADS 3: Equivocal
4️⃣ PI-RADS 4: High likelihood
5️⃣ PI-RADS 5: Very high likelihood
🧠 Teaching Pearl
PIRADS 5 lesions usually show:
✔ Obvious diffusion restriction
✔ T2 dark mass with clear margins or bulge
✔ Definite early focal enhancement
✔ Often capsular or NVB involvement
