Brugada syndrome is a genetic defect in cardiac sodium channels that can lead to ventricular tachycardia and sudden cardiac death, especially in young, otherwise healthy men. The syndrome is characterized by an incomplete right bundle branch block with ST-segment elevation in leads V1 to V3 on an ECG. There are three types of Brugada patterns:
Type 1: A coved ST-segment elevation of at least 2 mm, followed by a negative T wave in one or more of the right precordial leads (V1 to V3). The Type 1 pattern is diagnostic of Brugada syndrome.
Type 2: A saddleback ST-segment elevation of at least 2 mm with a trough displaying ST elevation of at least 1 mm, and then either a positive or biphasic T wave.
Type 3: Either a saddleback or coved appearance with ST-segment elevation of less than 1 mm.
Transient Brugada patterns may be induced by cocaine ingestion.
