The classic and most commonly available antidote for cyanide poisoning is a combination of agents found in the Cyanide Antidote Kit, though the modern standard of care often centers on Hydroxocobalamin (Cyanokit).
The primary goal of cyanide antidotes is to quickly interrupt cyanide's inhibition of cellular respiration by binding to it and removing it from the body. The traditional kit approach uses sodium nitrite and amyl nitrite to induce methemoglobinemia, which sequesters cyanide, followed by sodium thiosulfate to convert cyanide into the excretable thiocyanate. However, the current preferred first-line treatment is Hydroxocobalamin (Cyanokit), which directly binds to cyanide to form non-toxic Cyanocobalamin (Vitamin B12), a safer and highly effective method that avoids the risks associated with methemoglobinemia.
