There are different treatment approaches to help manage scoliosis, and each has different end goals and potential outcomes. The main types are traditional or conservative.
Traditional treatment options are more focused on watching and waiting; the main goal is funneling patients toward spinal fusion. Conservative approach is more functional and has the goal of trying to reduce curves so they do not become severe. It is more proactive, so the curve does not increase to where surgery is an option.
Each treatment uses a different type of brace. Traditional treatment uses a Milwake brace or Boston brace, which has the end goal of trying to slow and stop the progression. They have limited ability to reduce or try to make a curve smaller. CTLSO brace is used to treat primarily thoracic curves. A Milwaukee brace extends from the cervical spine to the pelvic area. They are still commonly used around the world. They aim to try to elongate the spine to decompress it. Although designed for full-time wear, it is difficult to tolerate and bulky. They can only be used in the growth phase where the adolescent is still growing, and their curve is still between 25-40 degrees. If your curve is less than 25 degrees, over 40, or stops growing, it is not recommended as it has no effect in trying to reduce the curve, only to slow down the progression.
Conservative treatment aims for modern corrective bracing like a Scolibrace are custom designed to fit the patient and are more comfortable to wear. The end goal is to reduce the curve at a structural level to impact and improve what a person already has. A corrective brace can be used at any age and any curve size. They are designed to correct the 3-dimensional components of scoliosis. As your curve gets smaller, it becomes easier to manage, and the brace becomes easier to wear.
The best brace is the one that is aligned with your treatment type and expectations. If your goal is to stop the curve from progressing, then a Milwaukie or Boston brace may be what you expect. If your goal is a correction to reduce the curve and improve the body function and shape, then a corrective brace or Scolibrace may be the best option for you.
Braces have varying names, but the person who is fitting your brace and ensuring it is aligning with your therapy and rehab is the person who is responsible for the outcome of your brace as that is the person ensuring the brace does what needs to be achieved. Make sure you seek someone with experience in what you're trying to achieve.
Corrective braces need constant adjustments in incremental patterns. At the Scoliosis Reduction Center, we add corrective forces to try to push the spine into a more straight position. The initial fit and design are important, but so is the proper management over time. In a corrective manner, we have to be more proactive with this type of treatment.
