After you get trapped in the underworld of the Vatican City following the bulldozer cave in, you need to find a way out. This sees you swimming to safety only to find blackshirt guards on the patrol to investigate. You'll first have to exit the Necropolis from here and then the Catacombs. Once you've done that it's a simple case of climbing out of the hole and you're free.
