Dr. Joseph Dearani discusses the treatment of aortic valve disease in infants and children. Visit http://www.mayoclinic.org/diseases-co... for more information on aortic valve disease or to request an appointment.
Aortic valve disease in infants and children is caused by changes in the aortic valve leaflets or changes in the geometry of the aortic root. The repair technique for aortic valve disease is tailored to each individual and can include the Ross procedure and autograft procedure. Indications for surgery or balloon valvuloplasty include a pressure gradient that exceeds 40 to 50 mmHg, the presence of left ventricular hypertrophy or strain on the echocardiogram, or evidence of left ventricular dysfunction with exercise testing.
The benefit of cardiac surgery in patients with aortic valve disease is to protect the heart from pressure and volume overload. However, the lack of a suboptimal valve substitute for children and the limited durability provided by the various aortic valve repair techniques makes the decision difficult.
