COMMON MISTAKES:
Not enabling SteamUGC file transfers in SteamWorks
Trying to upload a preview image too large in file size

Misc. troubleshooting:
Start off small if you're having issues. Start by uploading just a workshop submission with only the title configured. Next add description if it still works, then try adding the file, then image, etc.
May need to clear your UGC cache, I've never personally had this happen but apparently rummaging around in Steam files and clearing it may resolve issues uploading.
May need to increase the maximum file size in the SteamWorks settings
