PPS scores are determined by reading horizontally at each level to find a ‘best fit’ for the patient which is then assigned as the PPS% score.

Begin at the left column and read downwards until the appropriate ambulation level is reached, then read across to the next column and downwards again until the activity/evidence of disease is located. Repeat until all five columns are covered before assigning the actual PPS for that patient.

Reliable Evaluation Tool: PPS is a dependable method for assessing a patient’s physical and functional health, focusing on mobility, self-care, activity levels, food intake, and awareness. It’s particularly useful as an objective measure of decline.
Documenting Decline Over Time: By regularly applying the PPS, you can effectively chart the progressive decline in a patient's condition, which is essential for supporting the need for continued hospice, especially during recerts.
Predictive Insight: Lower scores on the PPS correlate with a shorter expected lifespan, making it a key tool in predicting whether a patient’s prognosis aligns with the hospice eligibility criteria of six months or less.
Informed Decision-Making: The PPS complements other clinical evaluations, offering a holistic view that aids in making well-informed decisions about a patient’s care.
Capturing Broader Declines: The PPS is not limited to physical deterioration but also captures cognitive and functional decline. This is especially useful in determining eligibility for hospice in patients with conditions like dementia or other neurological diseases.

Some agencies prefer the KPS which is also a commonly used tool to measure decline.
