Atrioventricular (AV) block occurs when there is a delay or interruption in the conduction of electrical impulses through the AV node or the His-Purkinje system. In a His bundle block, the impulse is blocked at the level of the His bundle, leading to a failure of conduction to the ventricles. This results in a characteristic "dropped beat" or a complete dissociation between atrial and ventricular activity.
In contrast, a Purkinje fiber block affects the terminal conduction system, leading to slower ventricular depolarization and a wider QRS complex. The mechanism behind these blocks often involves ischemia, fibrosis, or structural heart disease, which disrupts the normal electrical conduction pathway. Understanding the location of the block helps predict the clinical consequences and the appropriate management for patients.
