Phineas Gage’s tamping-iron injury in 1848 produced a focal, severe lesion of his left frontal lobe — damaging medial / ventromedial frontal cortex and substantial left-hemisphere white matter connecting frontal cortex to limbic and subcortical structures — and produced striking changes in personality, social conduct and decision-making without gross loss of intellect or memory. The case provided early clinical evidence that (a) the frontal lobes are important for emotion regulation, social behaviour and decision processes, and (b) damage to specific prefrontal areas (and their white-matter connections) can produce dissociation between preserved “cold” cognition and impaired affective/social cognition.
