So keloids what are they and how can we treat them keloids are excess growth of scar tissue which is beyond the sight of injury. Whenever our skin heals at times the programming goes wrong and the scar tissue starts overgrowing beyond the level of our skin. If it overgrows beyond the sight of injury it's called a keloid. If it just grows along on exact level of the injury it's called a hypertrophic scar. Keloids are generally occur after say an infection or after surgeries like a caesarean section or cardiac surgery. It could be after an inflammatory process like an acne or a blister. It could also happen after ear piercing or any other body piercings and sometimes sternal or chest keloids we notice because of genetic predisposition. There is no injury but the pressure that is caused by stretching of the chest can lead to a keloids in the chest area the keloids are very difficult to treat as I said because the programming of skin growth or collagen production in that area is malfunctioning. There are certain ways that we can keep it under control and those are the ones that I'm going to tell you you about today. The first the commonest thing that we use for a keloid which is just developing is topical treatments. And the first of it is silicon either in a gel form if it's a very small keloids or a silicon gel applicator in the form of a sheet which causes pressure prevents the keloids from growing up hydrates and moisturizes that area so that the keloid starts flattening. The other thing that we use is a corticosteroid cream now steroids by virtue of their effect on the cells in the particular area can reduce the excess production of collagen and thereby keep the keloid under control and try and heal it. If this doesn't work we go to the next stage where we start doing injectables. So one of the ways that we can do is inject a corticosteroid called triamcinolone into the keloid so that it starts working immediately causing flattening of the keloids as well as reducing the symptoms because sometimes keloids can be a menace. They can cause itching, pain, irritation and sometimes it can even get infected so treatment at the right time is very important. Very rare cases we inject an anti-cancer like a five fluorourasil into the keloids . If it's too large and not responding to a steroid if the injectables don't work then there are certain other options like cryotherapy we use liquid nitrogen at very very minus 40° C to completely freeze the keloid which flattens it as well as prevents it from growing. The only side effect here is that it can cause superficial skin pigmentation and you need multiple cryotherapy sessions which is why now as dermatologists we've all moved towards laser therapy because what laser does is it targets the specific blood vessels that actually nourish the keloid. So once those blood vessels constrict the keloid doesn't grow the collagen production reduces and that can flatten the keloid we also use certain microneedling and fractional laser techniques to remodel the collagen in the scar area. So once the collagen starts remodeling and becomes normal in production the keloid starts flattening. If all of these don't work and the keloid is too large then we opt for surgical options in rare cases where the skid is completely excised and shaved and then we use certain radiations, pressure bandages as well as certain creams to reduce and flatten that area so that the keloid doesn't grow back again. Generally a permanent treatment for keloid is not there but with all these methods we can flatten it and keep it under control. But once the skin changes its effect after a keloid. There will always be a scar that remains . But nothing to worry about keloids because now we have a range of treatments that can be offered to you depending on the stage of the keloid and the harm that it's doing to you and we can keep it under control.
