🌟 Reactive Postural Control for Older Adults
Reactive posture control is when our bodies respond automatically and quickly to unexpected changes in balance or the environment.
These strategies prevent balance loss by generating the correct motor response using the combination of the sensory systems of visual, vestibular, and somatosensory.
🌟 Why is this Important in Occupational and Physical Therapy
Decreased automatic postural control causes fall risk and there are many age-related changes to this strategy.
If our patient is lacking reactive balance they may not be able to maintain balance like others do while completing regular everyday tasks and activities.
For example, when someone with good control is walking on uneven surfaces and they lose their balance they will automatically make quick adjustments to correct their posture and therefore, prevent a fall. If our patient is lacking in that automatic response they may fall.
🌟 What are the Different Control Strategies
There are three balance strategies that we use most often in occupational and physical therapy for fall prevention.
These strategies help with balance recovery and are the ankle strategy, hip strategy, and stepping strategy.
What strategy is used is based on the severity of the loss of balance.
Even though these strategies may be used for different levels of loss of balance they still work together for the most part especially when there is a complex or unpredictable situation.
🎯 Ankle Strategy
This balance strategy is the first strategy used for smaller episodes of loss of balance.
This strategy uses small movements in the ankle joint to maintain the body's center of mass over the base of support.
Many times this strategy is initiated with a small push or a slight change in center of mass.
🎯 Hip Strategy
Hip strategies are used over ankle strategies when the body is slightly more displaced. With this amount of loss of balance, an ankle strategy will not be enough to prevent a fall.
This strategy makes larger adjustments in the hip position to center the body back over the base of support.
Besides the hip joint, the hip strategy also uses the muscle strength of the gluteus maximus, gluteus medius, and adductor muscles to help stabilize the body.
🎯 Stepping Strategy
Stepping strategies are an effective strategy to use when the body is significantly displaced from its stable position and the ankle and hip strategies aren't enough to regain balance and prevent falls.
The stepping strategy involves taking a step or multiple steps in the direction of the displacement to increase the body's base of support and regain balance.
This strategy is used most often when someone trips or slips. When the person trips or slips the body responds by quickly extending a leg and stepping therefore increasing the base of support and regaining balance.
