Описание: It’s a space that cares for patients beyond their medical needs. “The complex care center is a transitional clinic where we provide services to patients that recently were discharged from the hospital and don’t have access to their primary care provider,” said Lisa Libonati, a registered nurse with Lee Health.

Serving uninsured and underinsured, the Lee Health complex care center provides patients with a continuum of care. “The complex care center does anything that a primary care provider would do. We can also order infusion services, we have an infusion clinic. We have a clinic, where patients can be dosed appropriately and have their labs drawn. We also do wound care for patients,” she explained.

Located inside Lee Memorial Hospital, the complex care center provides patients with transportation, meals, and clothing. “We help address all of their needs, so it’s not just are they able to get the medications that they need, but are they able to afford the medication they need? Can we provide them with an alternative,” said Libonati.

The facility has proven to lower readmission rates and improve patient outcomes. “We have a lot of resources here that will help educate patients. It helps to motivate them. They feel empowered feeling like they can play a big part in making decisions in their healthcare. We see that patients actually do much better when they are able to get the services they need faster,” she said.
