This retrospective study of 70 patients with a surgically verified pancreatic cyst shows that endoscopic ultrasound (EUS) combined with contrast harmonic imaging improves the accuracy of the diagnosis. The major advantage of contrast harmonic EUS is its ability to distinguish nonenhancing mucus clots from enhancing true mural nodules and thus reduce the false-positive rate. If the presence of a mural nodule was considered to be indicative of high grade dysplasia or invasive carcinoma, the accuracy of contrast harmonic EUS (84%) was significantly better than that of standard EUS (64%).
