An air-fluid level on a chest X-ray appears as a horizontal line within a cavity or space, showing a separation between air and fluid. Air rises to the top, while the denser fluid sits below it, creating a distinct, visible line. This is possible only in cases where there’s both gas and liquid in a cavity, and the patient is in an upright or semi-upright position for the X-ray.
