Leaking Filling loop caused the Expansion vessel to Expand and leak

The filling loop is letting by no matter how hard you try to shut it using black levered ball valve. Also the PRV is completely blocked and water is not discharging, but luckly the expansion vessel started leaking. If the other pipe work or radiators started leaking due to high pressure, it could have been more costly. I end up replacing the filling loop, PRV and installed 25 litre new expansion vessel.
