Extra has different mechanics than main game. You and opponent can lose with 1 miss. You have a timer that indicates how long you need to survive. Opponent in that time is invincible. When the timer expires, the enemy will die quickly due to the AI's lack of intelligence and the massive barrage of bullets. The further you progress, the longer the timer is, but losing lives reduces it. In start you have 1 live and you can obtain lives with every 5 million points.

You will unlock Extra with Eiki after completing Extra as Komachi. Also after completing Eiki's route you will unlock main game characters in Extra and Extra characters in main game. There's no differences in plot, only in gameplay.

Enemies in Eiki's route are "fixed" random:
In Stage 1 you can meet Cirno or Mystia
In Stage 2 you can meet Cirno, Mystia or Lyrica
In Stage 3 you can meet Lyrica, Reisen or Youmu
In Stage 4 you can meet Reisen or Tewi
In Stage 5 you can meet Reisen, Youmu or Sakuya
In Stage 6, 7, 8 and 9 you will meet Marisa, Aya, Komachi and Reimu in that order
