This training package examines the importance of near-miss reporting. When near misses are reported, acknowledged, analysed and collated, lessons can be learned, publicised to the fleet and put into practice. In this way, the frequency of serious incidents can be reduced.
In the video, three near miss case studies are described, and the lessons that can be learned from them are discussed. It also looks at the many barriers to near-miss reporting and at how incidents involving third parties can be reported to the Confidential Hazardous Incident Reporting Programme (CHIRP).
