活動受限的距下關節會導致後足內翻,改變距下關節軸線相對於地面的關係,並阻礙足底壓力向內側的轉移。針對踝部訓練目的在於藉由牽長關節囊及伸展相關韌帶,來改善關節活動度(ROM),從而增加軟組織的延展性。改善關節活動度考量距骨、舟狀骨、骰骨、跟骨之間的連動、並增進關節功能,並消除持續性不穩定的感覺。這些益處可能與治療提升距下關節結構適應性與靈活度的能力有關,進而減少反覆性損傷的發生。
研究中,接受距下關節活動度提升的個案,與對照組相比,在步行 500 公尺後的疼痛明顯下降。此外,其單腳平衡能力(NLHSL、SEBT 與 AOFAS)也較對照組有顯著改善。這樣的改善可能歸因於距下關節僵硬度的降低,使後足對地面的適應性提升。適應性的提升有助於改善單腳平衡、降低步行時踝關節的能量消耗,並減輕疼痛。
“Subtalar Joint and Three Key Ankle Bones in Relation to Gait and Squatting”
Limited subtalar joint mobility can lead to hindfoot varus, altering the relationship between the subtalar joint axis and the ground and hindering the medial transfer of plantar pressure. Ankle-focused training aims to improve joint range of motion (ROM) by stretching the joint capsule and the related ligaments, thereby increasing soft tissue extensibility. When improving joint mobility, it is important to consider the coupled motion among the talus, navicular, cuboid, and calcaneus, in order to enhance joint function and eliminate the persistent feeling of instability. These benefits may be related to the treatment’s ability to enhance the structural adaptability and flexibility of the subtalar joint, thereby reducing the occurrence of recurrent injuries.
In the study, participants who received interventions to increase subtalar joint mobility, compared with the control group, reported significantly less pain after walking 500 meters. In addition, their single-leg balance performance (NLHSL, SEBT, and AOFAS) improved significantly more than that of the control group. These improvements may be attributed to a reduction in subtalar joint stiffness, which enhances the ability of the hindfoot to adapt to the ground. This increased adaptability helps improve single-leg balance, reduce ankle joint energy expenditure during walking, and enhance overall performance.
