This the the Living failures boss fight blood echo bug blood echo bug that will let you get 67 million blood echoes on the first play through of the game.
If you do it on new game plus it will yield enough to max out your character.
It only works if you have all three Heir Ruins equipped at the same time.
Then you have to kill The Living Failures with two successive visceral attacks. The attacks must be with in no more than 5 seconds of each other.
