Session Description
Children with medical complexity (CMC) have complex, chronic disease involving multiple organ systems, and are often technology dependent and frequently have significant disabilities. Families of CMC require intensive care coordination to meet their diverse needs. Care coordination is a patient and family centered, assessment-driven, team based activity designed to meet the needs of children and youth while enhancing the care giving capabilities of families. Family goals should be incorporated as key elements into shared care plans.
