I am a British national who has lived here all my life / I am a refugee etc. [delete as appropriate and insert more detail]

I am in priority need because [I have dependent children / have someone in my household who is pregnant / am vulnerable etc. - delete as appropriate and add in as much detail as possible. Insert contact details of support services e.g. GP, IDVA, social worker etc.].

Here is my 5 year address history: [insert as much detail as possible]

[explain why you became homeless]

[insert details of supporting professions]

[insert details of any disabilities that anyone in your household has as well as what the council needs to do to adapt their service]

[insert details of your income]

[insert details relevant to location e.g. place of employment, which schools your children are at, where your support network is, whether there are any areas where it is unsafe for you to be etc.]

[state whether you have any belongings that you can't protect and that you want the council's help to protect them]

I am finding the situation difficult and would appreciate it if all communication could be in writing so I can keep track of the process and make sure that I understand the information I need. I would also appreciate it if you could copy correspondence to [X - maybe a support worker or politician] to help me understand it.
