This achievement was added in patch 10.1.5. In order to get this achievement you have to kill Darkmaster Gandling and Kirtonos the Hearald.
Darkmaster Gandling
He is located in the very last room of the dungeon and will not show up until you have cleared all six rooms in the Headmaster's Study. You have to clear all 6 rooms of all mobs After you clear the last room, Darkmaster Gandling will summon in the center.
Kirtonos the Herald
Kirtonos can be summoned on the balcony of the Chamber of Summoning. In order to summon him you will need the Blood of Innocents. This item drops the from the top center room of the Headmaster's Study from Doctor Theolen Krastinov. Once you have the Blood of Innocents, go back to the brazier on the balcony and summon Kirtonos.
