The short answer is nothing but your car. First of all, let me say I’m discussing the process for the state of Arizona, particularly Maricopa county.
An independent company is contracted by the state to perform the emission test. It’s actually a pretty smooth and accurate process.
If you have a vehicle older than 1996, or a diesel, or a gross vehicle weight rating over 8,500 pounds, you simply drive to the station and you have to get into a specific lane for your vehicle.
If you have a 1996 or newer light duty vehicle, then before you go make sure your check engine light is not illuminated. If the light is on you will fail. Also the vehicle itself has to be ready. If the computer memory has been cleared and the vehicle has not been driven enough for the computer to monitor the emissions, then you won’t fail but you won’t pass either. The test won’t be performed and you will be asked to return at a later date. If your light is not on and your monitors are complete, then you can drive to the station and go through any lane. The OBD lanes may have longer lines but are still generally quicker.
