25 Questions You Need To Ask While Taking Medical History From Your Patients

Описание: History taking is an art, here we are sharing questions you need to ask while taking medical history from your patients:

Start with a smile, introduce yourself and confirm the patients name - age - gender

Ask an open question about the current presenting complaint followed by further analysis about site, onset, character, radiation, associations, what increases and what decreases

Explore if this is the 1st time for the patient to have this complaint and take more history if it is not the 1st time “History of presenting complaint”, and then ask related questions to have differential diagnosis in your mind

Ask about previous medical conditions like DM, HTN, Thyroid problems, heart problems ... So on

Ask if the patient is on any medications? Any kind of allergy to food or medications? Any running disease in their family? “Family history” any recent travel? “Travel history” Occupation and hobbies? And make sure everything is alright at home “Social history”

Ask about lifestyle habits including diet? Smoking? Alcohol intake? drugs? and physical activity? and ask about sexual and menstrual history if applicable.

Do a systemic review by asking about common symptoms of different body systems and exclude red flags to make sure everything else is alright

let the patient talks about their ideas, address their concerns and explore their expectations

Give the patient the opportunity to add any other info by asking open question like anything else? Then take consent for examinations
