This ultrasound clip shows multiple renal angiomyolipomas (AMLs) in a 56-year-old man, identified incidentally during abdominal imaging.
On B-mode, a large, well-defined, echogenic lesion (~3 cm) is seen arising exophytically from the renal cortex. Despite its size, the lesion appears somewhat ill-defined due to its increased echogenicity, which blends with the surrounding perirenal fat, a common diagnostic pitfall that may obscure lesion margins, especially in larger or exophytic tumors.
Color shows minimal or absent internal flow, consistent with the typical vascular profile of AMLs, although some AMLs may show more prominent flow if they contain larger vessels or aneurysms.
In addition to the dominant lesion, multiple smaller echogenic nodules are visible within the renal cortex, also consistent with angiomyolipomas.
AMLs are benign mesenchymal tumors composed of fat, smooth muscle, and blood vessels. The risk of hemorrhage increases with lesion size, particularly for tumors greater than 4 cm, and periodic monitoring is recommended for larger lesions due to this risk.
This is supported by clinical practice guidelines, which suggest considering intervention for lesions greater than 4 cm, symptomatic tumors, or those with intratumoral aneurysms greater than or equal to 5 mm.
