In this episode of The Nachiket Bhatia Show, Dr. T. S. Kler joins Dr. Nachiket Bhatia to discuss the evolution of cardiology in India, the ethics of modern medicine, and the lessons he’s learned over four decades in practice.
Dr. T. S. Kler, Chairman of the Heart & Vascular Institute at BLK-Max Super Speciality Hospital, New Delhi, is a pioneer in cardiac electrophysiology. With more than 35,000 life-saving procedures to his name, he introduced India’s first ICD, CRT-P, CRT-D, and His-bundle pacing. A Padma Bhushan awardee, he has authored over 150 research papers and holds fellowships from both the Royal College of Physicians (UK) and the American College of Cardiology (USA). He continues to train cardiologists across India and South Asia.
In this episode, Dr. Kler shares how he went from a small village in Punjab to becoming one of India’s top cardiologists. He cycled to school daily, became the first doctor from his village, trained at PGI Chandigarh and in the UK, and helped build Escorts Heart Institute into a global center. For him, learning always came before money - and ethics, empathy, and integrity are the foundation of great medical practice.
We also discuss:
Dr. Kler's efforts in training hundreds of cardiologists and his vision for stronger medical audits, universal healthcare access, and safer hospital environments. He stresses teamwork in medicine, the dangers of burnout, and why Indian doctors are globally respected. His advice to young doctors is clear: focus on skill and character, not shortcuts.
5 more unique insights from Dr. Kler:
Married against his father’s wishes - both he and his wife later received the Padma award.
Turned down a high-paying job in favor of learning under great mentors.
Believes senior doctors must stay humble and listen to juniors.
Advocates for merit-based medical education over reservation in PG admissions.
Additionally, Dr. Kler urges doctors to avoid unnecessary procedures and always prioritize patient welfare. He champions primary care reform and believes collaboration is key to safer, better healthcare.
Furthermore, he reflects on receiving the Padma Bhushan with pride. While supportive of AI in medicine, he believes it can never replace a doctor’s judgment. For Dr. Kler, real respect comes from doing the right thing - always.
A must-watch for medical students, young doctors, and anyone seeking real inspiration from a legend in Indian cardiology.
