Описание: Acrofacial vitiligo is one of the most difficult forms of vitiligo to manage. Simply because vitiligo occurring on the other parts of the body occur on other areas having hair follicles. Hair follicles are a good reservoir of melanocytes and these melanocytes help to repigment the patches. So response to treatment is good in the types of vitiligo. Acrofacial vitiligo involves the tips of the fingers the tips of the toes and the genital areas, so these areas are generally devoid of hair follicles and therefore have a poor response to treatment, if it is a very early stage. Then we first aim at making the disease stable and later we give medicines to prevent the spread of the disease and if the disease has already spread and reached a stage where it is not spreading any further then we call it as a stable vitiligo. Once the disease has become stable, then we can take up a patient who is not responding to medical therapy and to phototherapy for vitiligo surgeries. So generally our approach is first stop the disease, medical therapy for repigmentation, phototherapy for repigmentation and the last resort when the patient is not responding to anything else, then we take up the patient for surgery, even surgery there are different types of vitiligo surgeries depending on the area involved and depending on the cost factor and so may other things which we discuss with the patient when we provide them with options for treatment for vitiligo.
