In this short but critical training video, we cover how to identify, prepare, and perform a needle decompression for suspected tension pneumothorax in the field. Designed for medical professionals, students, and trauma specialists.

"Let’s talk landmarks for needle decompression. You’ve got two options."

First — the second intercostal space, mid-clavicular line:
Start at the suprasternal notch.
Slide down to the angle of Louis — that’s the second rib.
Now move laterally to the gap below — that’s your second intercostal space.
Line it up with the middle of the clavicle — that’s your target.
Insert the needle just above the third rib to avoid arteries, veins, and nerves.

Option two — the fifth intercostal space, mid-axillary line:
Raise the patient’s arm.
Find the triangle of safety — bordered by the pectoralis major, latissimus dorsi, and the fifth rib space.
Slide along the rib gap — insert just above the sixth rib.

📌 Use the anterior approach first. If that fails, switch to the lateral.
