It is well recognized that root cause is an essential element in Reliable Manufacturing℠. Unfortunately in many organizations, analysis is often where it stops and somehow the problems are either not eliminated or reoccur. In most organization the responsibility for root cause lies with engineering, which limits the application of the technique. This presentation lays out a practical problem solving approach that can be applied and used for any problem and by any level of the organization from top to shop. It also presents simple practical details of the forensics of failure including bearings, fasteners and materials failures. At the end a series of observations and challenges allow participants to return to their facilities and apply the lessons learned.
