Join Andrea Martin (South West Self-Management Program) and Nicole Seymour (Centre for Effective Practice) to discuss motivational interviewing in the context of fall prevention.

Attendees will learn about motivational interviewing as a collaborative patient-centred communication strategy to facilitate conversations about fall prevention and inspire patients to be curious about their health and behaviour change.

PRESENTERS:
Andrea Martin has spent the past 25 years working in health care from Long Term Care Nursing Home settings to active adult community programming. These have provided an opportunity to see the many experiences people face as they travel through their aging journey. With formal education in Kinesiology and Health Promotion, Andrea’s current role of Program Facilitator with the South West Self-Management program, allows her to share healthy aging skills to adults and health professionals. The South West Self Management program focusses on using Self-Management of your own health as a way to maintain control of your life, no matter what is thrown at you.

Nicole Seymour is experienced in both inpatient and outpatient hospital pharmacy, family health teams, and academia. Nicole’s experience in mental health, nephrology and geriatrics has inspired her passion for improving transitions in care and enabling patient-centered care. As an academic detailing pharmacist with the CEP, Nicole works with primary care clinicians to identify and develop evidence-informed, practical, and realistic strategies for enhancing patient care. She has supported nearly 200 clinicians with 600+ individualized best practice discussions.
