It's not just a decreased in blood flow to the tissue. Ischemia occurs when blood flow is inadequate to supply metabolic demand and removal of waste.

Ischemia does not necessary means that blood flow is below normal or resting level. If you are out exercising intensely and your heart is pumping five times as much blood as usual, and it requires a tremenedous amount of oxygen to do that. You can have normal coronary blood flow, which is inadequate to meet the exercising condition. That is still ischemia.

So ischemia should be correctly defined as: insufficient blood flow to meet the metabolic demand of the tissue.
